Do we take your insurance?

We have contracts with Aetna, Blue Cross Blue Shield, CIGNA, Health First, Humnana, Medicare, United Healthcare and more.

 

Call the office to schedule an appointment.

To become a new patient you must call the office at (321) 768-7677 to schedule your first appointment. You will be asked to provide some basic personal and insurance coverage information, along with your e-mail address.

 

Register with your PIN.

After scheduling your first appointment, we will send you a one-time PIN enabling you to Register today!

Please complete the following steps prior to your first appointment: Create your Portal login ID and password, verification of your identity with the PIN, submit your preferred method of contact, fill in any remaining demographic information, and complete your New Patient  & Medical History Forms.  If you do not complete the forms online, you will be asked to complete them in our waiting room and we ask that you arrive 15 minutes early for your appointment to do so.
